Note:  The medical providers at CCFM prescribe narcotics and other controlled medications in a judicious and conservative manner.  If your previous medical provider prescribed narcotics or controlled medications for you, it is not guaranteed that your new provider at CCFM will continue to prescribe that medication.  Please take this into consideration before deciding to become a patient at CCFM.  We strongly suggest that you read our 2025 Policies & Procedures carefully before you enroll.
